Capstone Safety Assignmnent

This assignment addresses the following CLOs

CLO # 2 – Design an individually developed team project that has a focus designed to improve patient/community health outcomes.

CLO # 4 – Analyze readings in collaboration with class peers online, focusing on specific course content.

CLO # 6 – Determine nurse sensitive or other appropriate outcomes for selected project.

This assignment addresses the following PLOs

PLO # 2 – Participate in patient safety and quality initiatives within the context of an interprofessional team.

Other PLOs may be addressed as well

The Capstone Projects requires that students design an individually developed team project that has a focus designed to improve patient/community health outcomes. The entire project or elements of it may involve safety for patients and/or communities.

1. Identify and describe one significant safety concern in the clinical site in which you are carrying out the Capstone Project (or another clinical site). Include the complexities of the setting and/or the concern such as: pertinent history; political, social, financial and/or other constraints/limitations; resources available or needed; limitations of human factors.
If the focus of your capstone project is not directly related to a significant safety issue you can complete this assignment by identifying one significant safety concern for another clinical site that you have identified such as a place of employment. If you do not use your capstone project site you should email your NURS 499 instructor with your alternate site for this assignment.
2. Describe the nurse sensitive or other outcomes that are appropriate for the safety concern you have identified. If you have designed your capstone project specifically related to the safety concern described in # 1 above you can use those outcomes.
3. Describe one intervention related to the safety concern. This can be part of your capstone project or something the agency has initiated.
4. Explain how this intervention will be evaluated and any evaluation data that is available at the time of this assignment.
5. Describe how the intervention can be related to one of the professional organizations that are concerned with safety (ANA, WHO, Joint Commission, Medicare, Patient Safety Movement, AHRQ).

The assignment is a 4 to 5 page paper that is submitted to SafeAssign by the due date. The paper will be graded according to the rubric. This paper is also to be posted in the student’s ePortfolio.

Texts for Week 1 and 2
Chapter 15 pp 429-437 in text – Roussel, L. Harris, J., Thomas, T (2016) Management and Leadership for Nurse Administrators (7th ed.). Burlington, MA. Jones & Bartlett Learning.
Chapter 1 & 7 in – Harris, J. L., Roussel, L., Walters, S. E., & Dearman, C. 2nd Ed. (2016). Project Planning and Management: A Guide for CNLs, DNPs, and Nurse Executives. Sudbury, MA. Jones & Bartlett Learning

Text Book – Chapter 4 page 68 & Chapter 5 in Roussel, L. Harris, J., Thomas, T (2016). Management and Leadership for Nurse Administrators (7th ed.). Burlington, MA. Jones & Bartlett Learning.

Topic I chose:
Empowering patients with central line venous catheters, utilizing Evidence-Based education.
During my first four weeks of clinical hours I found there to be an educational deficit in English and Spanish speaking patients in respect to the care of their CVC device when receiving inpatient dialysis at Flushing Hospital Medical Center.
Please incorporate the hospitals mission statement in the safety assignment, here is a link to help;
https://www.flushinghospital.org/general-information/mission-and-vision

Example of a similar project I found online that I will use to guide me on this project
Please use any of this information to guide you
– As a med-surge Registered nurse I have been assigned to work on the 3 West unit with a preceptor to access the needs of patients with Central Venous Catheters receiving dialysis to meet their needs at Flushing Hospital in Queens NY which is a community-based hospital. I have collaborated with the Nursing Education department, unit staff members and my preceptor to work up a plan to educate these types of patients on ways to prevent CLABSI (Cental line associated blood stream infections). I have also used the hospital library to research evidence based information for this project.
Example of a similar project I found online that I will use to guide me on this project—

CVC Patient Survey, Purpose and Methods
To identify opportunities to improve overall CVC care and reduce CLABSI incidence and severity, dialysis patients surveyed to assess: their own knowledge of CVC home care and infection Signs&Symptoms / 10-question survey based on existing survey tool, administered verbally and recorded manually
Demographics  n = 25  Age  mean: approx. 58 yrs  range: 50-80 yrs  Gender  7 male  6 female  Months since CVC was placed  mean: 1.7  median: 2  range: 0 (as short as <1 week) to 5 (>4 months, <5)
QUESTIONS ABOUT CARE
 Have you noticed any differences in how different staff at CCC care for your CVC line? If yes, what difference?  Only 2 yes responses, neither including safety concerns  Consistent care across nursing staff  Reinforces that no one person is responsible for multiple PICC-related CLABSIs

When staff use an alcohol wipe to clean the hub (connection points) before connecting anything to the line, do they scrub that connection point, or wipe it off?  2 responses that indicated “wiping” as opposed to scrubbing  Has anything about the way the staff here at Flushing Hospital cared for your CVC ever concerned you or struck you as unusual? If so, what specifically?  2 yes responses

Patient Knowledge and Home Practice Questions ( 10 Question Survey)
1) How did you learn about how to care for your CVC at home?  12 responses that they learned by instruction from a nurse  2 responses that they also received written material  2 responses included mention of also relying on a caregiver for CVC care at home  1 response that no instruction was received at all  Suggests that written material should be offered to take home
2) If the dressing became loose or opened on one side before a scheduled dressing change, what would you do?  8 responded that they would call Flushing Hosp or another specific hospital or clinic that was closer to them or had to do with their PICC placement or care  5 responded that they would (or would also) go to CVC, another specific hospital or clinic that was relevant to their CVC or would contact an RN caregiver  1 responded that they would contact their doctors office  1 responded that they would tape it up themselves  1 responded that whether they went to Flushing Hospital, would depend on if it appeared infected or how close they were to their dressing change appointment

3) What do you do when showering or bathing?  12 responded that they cover it in saran wrap, press’n’seal, an arm-length glove, or a plastic sleeve  1 replied that they also typically take a bath and when they do that they just hold their arm out of the water  1 responded that they only take a sponge bath

4) Do you know what symptoms you might have if your CVC line was infected? If so, what are they?  9 responses of redness at the site  7 responses of pain or soreness at the site  2 responses of drainage at the site  3 responses of swelling or warmth at the sight  5 responses of fever  2 responses of malaise  1 response of “No, I really don’t.”  8 surveyed patients replied with 2 signs/symptoms or less  8 surveyed patients included only site-specific signs/symptoms
5) I will MAKE 6 More questions before or after these examples, coordinate responses to the sample size I will use. Keep in mind this is being done during a four month semester. I am doing 10 clinical hours a week with inpatient dialysis clients that are with us for an average of 1-2 weeks before discharge. These patients receive 3 dialysis sessions a week Monday Wed and Friday, my professor suggested I do an initial 10 question survey and then reassess during one of their future sessions to gather analysis regarding improvement in patient education that will empower patients to prevent CLABSI infections, reducing hospital costs and readmissions.

I will be generating a 10 question survey in PDF form for patient knowledge assessment
I will be providing a Patient education Pamphlet handout
I will then reassess their knowledge before discharge, this information will allow me to conclude the success rate and outcome of my project.

Final concept paper using Theoretical concepts in nursing informatics

Final Concept Paper
Purpose: To broaden the student’s perspective of the role of technology in the workplace.
Objectives:
1. Apply Theoretical concepts
2. Identify the information technology needs of student’s current workplace
Description: This experience will be organized around professional values, principles of social
justice, advocacy, personal interests, nursing informatics theory and scholarship. You will use
the community agency technology checklist to assess the information technology at your current
workplace.
Background: While the need for information is great, shortages of time, staff, money, and/or
expertise make it hard to use information technology effectively. The result is often a vicious
cycle in which staff time is spent compiling information manually. Observe how your
organization could benefit from the implementation of a new technology. Interview some key
stakeholders in the organization and summarize your findings using the checklist below as a
guideline. It is NOT necessary to include every item on the checklist in your paper.
1. Describe in detail the planning process and the implementation plan. Describe
positive as well as negative aspects and suggest what should have been done
differently (what might the limitations be?).
2. Design a formative and summative evaluation plan for your intervention. While a
formal summative formal evaluation may not be feasible please give a subjective
view of anticipated outcomes
Technology Checklist
A Technology Plan is really just another form of Strategic Planning. A well-done plan can
provide insight into the organization, including how to strengthen and improve the way the
organization accomplishes its mission. A well-done plan may also open up new opportunities to
find resources the organization needs, not only to acquire the technology it may need, but to
serve their community better, faster and in new ways they may not have thought of before.
PART I
1. What do you see as the most pressing needs of the organization that technology might
address?
2. Why/how do you think computers can help?
3. If all computer systems were magically working and adequate tomorrow, what would change
in the organization?
4. Who at the agency has been involved in planning for technology staffing, training and
purchases?
5. Who at the agency has been involved in day-to-day computer troubleshooting and
maintenance tasks?
6. Who will be involved in the implementation of new technology efforts?
7. Are staff members able to use the technology that is crucial to their efficiency and to the tasks
they need to accomplish?
8. What type of training have staff members completed in the past? How useful was it?
PART II
Local Area Network (LAN) Assessment
The agency might use these questions to help them think through their local area network needs.
Do they currently have a local area network? If you do not have a LAN:
1. What would a LAN enable you to do that you can’t do now? For example, a LAN might:
* Allow staff members to access each other’s files on the computer
* Enable you to share Internet access across all the computers on the LAN
* Allow staff members to use one central database
* Allow you to set up an automated backup system for all computers on the network
Resource Issues
1. What is your organization’s budget for computer systems? Have you explored fundraising
options?
2. What is your budget for technology support and maintenance?
3. How much money would you estimate your organization has spent on your computer systems
(hardware / maintenance) in the past year?
4. How many computers are in the main site of your agency? Do you have an inventory of your
computers?
5. Are your systems standardized or wildly different from each other? For example, do you have
a combination or Macs and PCs, or a combination of newer and older systems? If they are
different, does it cause problems? How serious are the problems?
6. Do you have any satellite sites? How many computers are at each satellite site?
7. Do all staff that needs computers have them? Are the computers adequate for the tasks that
staff need to perform?
8. Generally, are your computers new enough to run most of the software you need to run?
Agency Needs
1. What kind(s) of word processing software does your organization use?
2. What are the main types of documents that your organization creates with your word
processing program?
3. Does everyone on staff with a computer use word processing?
4. Is everyone on staff able to create the documents they need to make?
5. Do staff members need more training than they’re getting in doing word processing?
Accounting
1. Does your organization have a person on staff doing your accounting? If so, is your
accounting system computerized?
2. If the accounting system is not computerized, do you think it should be? Why?
Internet Connectivity Assessment
Use these questions to help you think through your Internet connection needs.
Do you currently have Internet access in your organization?
If you do not have Internet access:
1. What tasks could you do using the Web that you can’t do now?
2. Do you currently have an internal office email system? If so, is it adequate, or do you need to
email people outside your organization as well?
3. Who among your staff needs to do research on the Web as part of their job?
4. What is your budget for establishing a connection to the Internet? Have you explored
fundraising options?
5. Do you need all your computers to connect to the Internet, or just one or two? Do you
currently have a local area network that you could connect? Do you have plans to install one?
6. How many phone lines do you have in your office? Are any of the
se lines used for multiple purposes (e.g. for both fax and data)?
7. What services do you need from an Internet Service Provider?
Does the organization have a database?
If you do not have a database:
1. How do you currently store data such as contact information or information about clients and
projects?
2. Is your method of keeping data efficient? Is it easy to share data among different staff
members?
3. What is your budget for a new database?
4. What type of data do you want to store in the database?
5. What tasks will you use the database for? For example, do you want to retrieve information,
sort data or print reports?
Does the organization have a website?
In this section you should ask yourself what the organization is trying
to say on their site.
1. What is your message?
2. Who are you targeting? Who is your audience? Who would use your site? Why would they
come to your site?
3. What is the budget available in your organization for web development? How much for setup
and how much for maintenance? What are the ways that you could get funding for your website?
Do you know what type of website you can afford (large-scale database type, or online brochure
type?)
4. How can you present your point of view effectively? What kind of voice is appropriate for
your audience? What kind of content will be available? What are your featured categories?
Should your site be consistent with other collateral materials in your organization? Do you have
several other sites that you want to emulate?
5. What will your website accomplish? Will your website be a research or a development tool?
What would your site’s main use be? How will it improve what you do have? What will be the
benefits of online exposure? What will it bring you that you don’t already have?
Use these questions to help you think through the organization’s staff training needs.
Staff /Skill Profile
1. Who is the target audience? What are their skills and motivation level?
2. What training has staff requested?
